Complete an Enrolment Form

You have been directed to this page to complete/download an enrolment form for each person you have booked into a KMR Safety First training course.  Your booking is not fully confirmed until payment has been received and enrolment forms have been completed and returned to us.

To complete your enrolment form, click on the link below, complete the form, and click on the SUBMIT FORM button in the top right hand corner of the screen.  You can then also save your own copy of the form to your computer.

To complete for more than one person, save the form multiple times using each persons name as the file name, then complete and submit each individually.

NOTE: Should you have an older version of Acrobat Reader you may not see the SUBMIT button.  In this case simply save the form to your computer, complete on screen, save and email to us.  Alternatively, you may like to print and complete manually, then return to us by:
